Please note, We collect different Personal Data and use it for different purposes depending on whether You are accessing the Service and Site as a Guest, a Healthcare Provider, or a User. Please see Our definitions section below for a clear definition of those roles and click on the section of the Privacy Policy that applies to You in the role that You are using the Site or Service to get more information. Some of our Services, including the ability to book an appointment through our Services, are limited to Users with a Volodoc account. In addition, the definition of Personal Data can vary by jurisdiction. If We collect information from You that is considered Personal Data in Your jurisdiction, We will treat it as such in accordance with this Privacy Policy. Our privacy practices are subject to privacy laws in multiple jurisdictions. You may see references to practices that only apply to individuals living in certain U.S. states.

Your privacy is important to Us and We follow the following principles:

• For Healthcare Providers, We store information about Your practice, including Personal Data, to allow You to book appointments for health care services with Users. We are a third party marketplace where Healthcare Providers voluntarily provide us with their credentials, services they provide and geolocation for Guests and Users to browse on the Website. Your use of health-related information is not stored when You are using the Services. You can delete or modify any Personal Data in your account at any time. We may use your Personal Data to market Our services to You both directly and through targeted marketing.
• For Guests, We do not retain or store any health-related information even if you use it to browse for Healthcare Providers while browsing the Website. If You apply for a job with Us, We collect Personal Data related to Your job application.
• For Users, We collect limited Personal Data that you provide when setting up Your account to allow Healthcare Providers to book appointment with you and We do not retain or store any health-related information. Your inquiries and communications with Healthcare Providers are not stored when You are using our Services. We do not sell or share your Personal Data.

Any use of the Services is subject to the Agreement (as the term “Agreement” is defined in our Terms of Use located at http://volodoc.com/Terms, which incorporates this Privacy Policy).

Definitions. It would be helpful to start by explaining some defined key terms that are used in this policy:

Account	a unique account created for Healthcare Providers or Users to access Our Services.
Cookies	Cookies are small files that are placed on Your Device by a website, containing (among other things) the details of Your browsing history and/or connection information when using the Website.
Device	means any device that can access the Services such as a computer, a cellphone or a digital tablet.
Geolocation Data	IP address-based location information and zip code, city or address as provided by You
Guest	visitors to our Site and users of our Services who do not have an Account (for example, if you visit our websites or download our mobile application without creating an Account or to run Healthcare Provider queries).
Healthcare Provider	doctor, medical group, or other healthcare professional who has signed up as a “Healthcare Provider” to use Volodoc Services.
Services	Volodoc software, applications, and services provided to Healthcare Providers for use with their Users
Usage Data	Data that is collected automatically and generated by the use of the Services or from the Services infrastructure itself, including: o When You access the Services by or through a Device, the following may be collected about Your Device: its type, unique ID, IP address, operating system, Internet browser, possibly other identifiers; and o Information that Your browser sends whenever You visit this Website or when You access the Services by or through a mobile device, such as the pages of the Website that You visit, the time and date of Your visit, and the time spent on those pages.
User	Individuals who are interested in identifying Healthcare Providers and have signed up as users of the Services, including the ability to book appointments with Healthcare Providers.
Web Beacon	Small electronic files (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit counting of users who have visited web pages or opened an email and for other related website statistics (for example, recording the popularity of certain website content and verifying system and server integrity)

If You are a Healthcare Provider

This section applies only if You sign up and have been given access to a Healthcare Provider Account. Some features are not activated automatically and require explicit authorization by You.

What Personal Data Do We Collect?

The Personal Data that We collect from You depends on how You interact with Us, the Websites, and the Services. The following includes the categories and types of Personal Data We have collected from Healthcare Providers within the last twelve (12) months:

• Identifiers:

  • Name
  • Phone number
• Professional/non-public education information:
  • Specialty
  • Position or title
  • National Provider Identifier (NPI)
  • States licensed to practice
• Geolocation Data
• Commercial Information:
  • Records of the Services Purchased or Used
• Internet Activity:
  • Usage Data such as browsing history and search history on the Website
• Content Not Directed At Volodoc, which may include Sensitive Personal Data that your Patients or you provide:
  • Information regarding the health condition a User that are collected and transmitted through the Services but not stored or retained by Volodoc.

What Personal Data do We intentionally NOT Collect?

Although Volodoc Services are targeted to the healthcare community, Volodoc does not collect or store any health or sensitive information You disclose while searching for or communicating with a Healthcare Provider. Any health or other sensitive information You disclose or share with a User is encrypted over a secure channel using the HTTPS Internet protocol with TLS encryption that are captured and stored for a transitory period on Our servers as further described below.

What Personal Data Do We or our Service Providers collect, securely transmit, and delete?

Invitations

You may invite Patients to Sessions by the following methods:

• Email sent by the Service via a third-party Service Provider
• Email sent by Your computer or smartphone email system
• Mobile text message via a third-party Service Provider
• Calendar

Any information You enter into the invitation sent by the Services will be used to create and send the message. Once that task is completed, the information is no longer used and will be permanently deleted by the service provider in the ordinary course.

How Do We Collect Personal Data?

When using this Website as a Healthcare Provider, We collect Personal Data:

• Directly from You:
  • When You sign up for the Services;
  • When You log into or make changes to Your Account;
  • Details of transactions You carry out through the Website; and

The Personal Data shall be collected for the business purpose of identifying You to the Patients using the Services and to assure full operation of the Services during all Sessions. Volodoc may use Personal Data for the following purposes:

• To provide and maintain Our Service, including to monitor the usage of Our Services.
• To manage Your Account: to manage Your registration as a user of the Services. The Personal Data You provide can give You access to different functionalities of the Services that are available to You as a registered user.
• To notify You: If You enable email or text notifications within the Services, We will send You an email notifying You about a User’s interest in contacting You. To opt-out of these notifications, please log onto Your Account and update the notification settings as needed. We also notify You about appointments made, and send reminders about appointments as well, and request reviews on Users.
• To contact You: to contact You by email, telephone calls, SMS, or other equivalent forms of electronic communication, such as a mobile application's push notifications regarding updates or informative communications related to the functionalities, products or contracted services, including the security updates, when necessary or reasonable for their implementation.
• To inform You: about news, special offers and general information about other goods, services and events which We offer that are similar to those You have already purchased or enquired about. By using the Service, You allow Us to contact You by email or within the Services regarding the Services' features, changes, maintenance, and other products that may be introduced by Us from time to time. You have the right to opt-out of receiving such information by clicking on an unsubscribe link embedded within the email message.
• To manage Your requests: To attend and manage Your requests to Us.

• With third-party advertisers: We use Personal Data that We collect and that You provide to Us to deliver advertisements according to Our advertiser's targeted audience preferences. What Personal Data Do We Collect?

The Personal Data that We collect from You depends on how You interact with Us, the Websites, and the Services. If you are a Guest that is browsing the Website and only searching for potential Healthcare Providers, we do not collect or store any Personal Data other than Your Usage Data relating to your activity on the Site unless you are a job applicant (see below). Guests my elect to become Users by setting up an account and self-reporting certain required Personal Data prior to being able to set up appointments with Healthcare Providers. The following includes the categories and types of Personal Data We have collected from Users while setting up accounts within the last twelve (12) months:

• Content not Directed at Volodoc, which may include Sensitive Personal Data that you provide:
  • descriptions of Your health condition provided by You to identify Healthcare Providers that transmitted through Our servers (see disclosure below) ***
  • Appointment date/time, provider information, appointment procedure, whether you are a new patient of a particular Healthcare Provider***
  • Heath information such as health conditions, Healthcare Providers visited, reasons for visit, dates of visit, medical history, and other health information you provide Us***
  • Health insurance information, such as: insurance plan, member ID, group ID, payer ID***
  • Sensitive demographic data, such as gender identity (specifically status as transgender or non-binary), race, sexual orientation. ***
• Internet Activity:
  • Usage Data such as history of access to the Website.

* User name entered by You to enter the Services. ** Collected if your Healthcare Provider has opted to use these to notify you. *** Collected and transmitted by You through Our Services and not stored. We have no control over the Personal Data You disclose through Your searches nor do we store such information on our systems. A User must create an Account or register with Volodoc in order to use the Services. A Healthcare Provider will email or text You based on Your request. Your name is only seen by the Healthcare Provider and used for an initial identification. If Your Healthcare Provider has used Our Services to text or email Your appointment, then We will have sent certain information to a third-party for transmission for which Your data will be permanently deleted by the vendor in the ordinary course. If You have called us, chatted with us, or emailed Us for customer support, We will have a record of this Communication.

Job Applicants

For Guests or Users that are job applicants, the following includes the categories and types of Personal Data We have collected from Guests within the last twelve (12) months: identifiers/professional/on-public educational information: If You apply for a job with Us through the website We may collect the following information: name, email address, mailing address, phone number, resume, LinkedIn profile, state of residence, desired salary, experience level, and sample work product.

What Personal Data Do We intentionally NOT Collect or Retain?

Although Volodoc Services are targeted to the healthcare community, Volodoc does not collect or store any health or other sensitive information You provide to a Healthcare Provider. Any health or other sensitive information You discuss or send the Healthcare Provider is sent over a secure channel using the SSL Internet protocol.

How Do We Collect Personal Data?

We collect Personal Data:

• Directly from You when You set up an account and input the required account information or if You contact Us for information about the Services or the Website, including from customer service.
• Automatically, through Usage Data we collect from usage of the Website.

The Personal Data shall be collected for the business purpose of identifying You to the Healthcare Provider using the Services at Your direction. Volodoc may use Personal Data for the following purposes:

• To notify You: If Your Healthcare Provider enables email notifications within the Services, We will send You an email notifying You when it is time for Your appointment. Your Healthcare Provider may also enable You to be notified by text message should You provide Your mobile number. To opt-out of such notifications, please contact Your Healthcare Provider.
• To manage Your support requests: To respond to Your customer support requests to Us.
